WHY CONVERSATIONS FOR LIFE?

Studies show that most of us expect our families to carry out our wishes about end-of-life care — yet 75 percent admit we have never clearly articulated what those wishes are. And our families are afraid to ask.

Legislation in some areas requires healthcare professionals to ask about our wishes, yet these conversations require education and training to best support patients and families.

• 50% of us are caring for an elderly parent or loved one.

• 70% of us die in institutions when 80% of us want to live our last daysat home.

• How can we make a difference for ourselves and those we care for?

• Why must we talk about this now?

• Traditional planning and forms will not ensure compassionate care.

• Conversations-can.
